Level D means
the deficiency found was NOT a pattern or widespread, instead it
was considered an ISOLATED incident. Level D also means NO
actual harm was caused to any patient. A nursing
assistant left a patient sitting alone in a shower room for a quick moment
while she grabbed an extra towel from the linen room right next to the
shower room. There was no incident or injury caused to the patient.
Corrective action and re-education was immediately completed with all
nursing staff.

3) Call the Family Caregiver Support
Network (State of WI authorized call-in center) 1-800-449-4481, to speak
to a counselor and receive information on facilities.
4) Ask to view a facility’s letters,
satisfaction surveys and testimonials from patients and family
members. 5) Ask those you know
what they’ve heard about the facilities you are considering.
